Every woman who has experienced childbirth, either vaginal or
Caesarean, should get pelvic floor physical therapy to regain the health of their pelvic girdle/ core. It blows my mind that if any other musculoskeletal tissue was stretched or damaged beyond their normal state physical therapy is always recommended as part of the care plan.
OB/GYN are so good at taking care of the baby and Mom for the first three trimesters, it just makes me sad that more often than not the 4th trimester does not get the same attention towards mama as the previous 3**

I’m a women’s health physical therapist new to Wilmington but not new to working with clients to achieve their health and wellness goals. With 20+ years’ experience in orthopedics, manual therapy, neuromuscular education, teaching and training, I bring a holistic perspective with an integrated systems approach to my client’s treatment needs. I treat women throughout their life cycles including adolescences/athletes, pre- and post-partum, pregnancy and peri /post menopause. Each phase of women’s life is unique with its own set of challenges. We work together to achieve the goals and desires you set for yourself.

ARTICLE HIGHLIGHTS ON WOMENS HEALTH AND LEVELS- IS INTERMITTENT FASTING GOOD FOR WOMEN?
Women may experience different outcomes with intermittent fasting than men, including fatigue, weight gain, hunger, and inflammation, due to hormonal fluctuations.
Fasting can increase cortisol and blood sugar levels in women, disrupt thyroid function, and affect sex hormones like estrogen.
To make intermittent fasting work, women should opt for daily time-restricted feeding of 12-14 hours, avoid longer fasts from days 15-28 of their cycle, and increase calories and carbs during the luteal phase.
It’s important for women to recognize their baseline stress levels and sleep needs when attempting intermittent fasting.
A more flexible, nuanced approach to intermittent fasting is often required for women due to hormonal cycles and differences in metabolic response.
How to relax a tight pelvic floor
Do you feel you have been over doing your Kegel exercises and maybe feel too much tightness in your pelvic floor or difficulty passing gas? It’s much easier to release tension or tightness in the pelvic floor if it’s a new symptom and you know you’ve been overdoing your Kegel exercises and maybe haven’t focused enough on the relaxation phase or given yourself enought“off’ days from doing the strengthening exercises.
If you don’t have control of gas or have difficulty letting, go easily it can be a sign of pelvic floor tightness. Another sign could be burning or pain during intercourse or incontinence.
First, take a break from the Kegel exercises. In fact, these issues could be from pelvic floor hypertonicity or overactive pelvic floor muscles and strengthening exercises would be contra indicated. Instead, work on relaxing and “letting go” of the pelvic floor. Use your breathing to help. Inhale into the back of your pelvic floor. What does this mean? Obviously, the breath stays in the lungs but as the diaphragm contracts and moves into the abdomen the pressure it places in the pelvic bowl can be directed into the back of the pelvic floor. Some positions to do this in are child’s pose or quadruped.
Massage can also be a great treatment solution just like for tight shoulders or neck muscles. Manual therapy can help stimulate muscle relaxation and your pelvic floor is just like the skeletal muscles in all the other areas of your body. You may want to see a pelvic floor specialist do get an internal pelvic floor massage because it’s a more direct way to get to the deeper layers of the pelvic floor muscles or to get a more advanced form of external manual therapy for the pelvic floor.
To try from the outside on your own try using a small ball (ie. pinky ball) or another size ball that might work for you. You may want to clear this with your physician if you have any contra indications for massage or manual therapy like being on blood thinners or have a connective tissue condition. Sit on a supportive surface /chair and find your sitz bones. Place the ball inside the sitz bones, not on the tail bone and slowly sink in and gently r

Who is a candidate for Pelvic Floor Dysfunction?
Most people believe that PFD only happens to women during and after pregnancy. This can’t be further from the truth.
First of all, ANYONE, male OR female, can potentially have this condition. The pelvic floor is a set of muscles which function to provide stability and support of the pelvic girdle and it’s contents. These muscles communicate with nerves that orginate in the lumbar and sacral regions of the spine, and therefore any injury affecting the spine or MOTION of the spine can have an affect on the nerves and the Pelvic Floor muscles.
Anyone with a history of low back, hip, SI (sacroiliac joint) pain or knee pain can potentially be susceptible to this condition.

What do we mean by Pelvic Floor?
There is a lot of talk about engaging “Pelvic Floor” muscles for core support, and doing Kegel exercises to strengthen those muscles, but do you really know what that actually means?
The pelvic floor is 5 layers of connective tissue, 3 of those layers are skeletal muscles that function to support the structure of the pelvic girdle (including the SIJ , hip and pubic joints) as well as supporting our pelvic organs such as the bladder, uterus, vagina and colon/rectum. These muscles form a left and right sling type muscular band that supports our organs from the bottom hence the term “pelvic floor muscles”. Without their proper strength and function we can not obtain functional strong core strength.
We engage in many activities throughout our daily life that challenge our pelvic floor, some are as follows:
-running
-high impact sports
-lifting or weight training
-exercise
-pregnancy
The pelvic floor is the foundation upon which the remaining core muscles gain support. They are necessary for continence, sexual function and normal bowel movements.

Do you know that 1 in 3 women suffer from incontinence?
How many of you have heard that’s it’s “normal” to be incontinent as we age? You may be surprised to learn that as much as it is deemed common, it IS NOT a normal part of aging.
Your bladder is a smooth muscle that sits in the pelvic girdle behind the pubic bone. It needs to be supported in order to properly fill without leaking. The muscles of the pelvic floor function as this support. These muscles are located at the base of the pelvis between our “sit bones” and can become weak or tight depending on excessive stressors, such as pregnancy or heavy weight training/lifting or conditions causing pain in the low back, hip or neighboring areas. If these muscles become weak or tight we may loose our ability to maintain our bladder control.
